Bilingual Field Care Coordinator – LVN / LSW

HealthCare Support is actively seeking a Bilingual Field Care Coordinator – LVN / LSW to fill an opening with a Managed Care Organization in Laredo TX area.

Daily Responsibilities: 

  • Will have 3 days of visits and 2 days of documentation weekly
  • Will have a caseload and work assigned tasks
  • Will complete H2060 assessment
  • Fill put required forms for each assessment
  • Make follow up calls
 

Required Qualifications:

  • 2+ years of experience in managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions
  • Must have clear and active Texas LVN/LPN or LSW/LBSW license
  • Bilingual in English and Spanish – fluent verbally and written
  • Clear and active driving record, reliable transportation and insurance
  • Know/Understand Medical Terminology and Disease process
  • Microsoft Outlook, Teams, Excel, Word, 50 wpm typing speed

Pay Details: $24 per hour – $26 per hour
