Field Case Manager / Registered Nurse

RN Field Case Manager 

HealthCare Support is actively seeking a RN Field Case Manager to fill a permanent  opening with a well respected healthcare provider in Willmar, MN and surrounding areas.

• Mileage Reimbursement – 70 cents/mile

Pay:  $34.56 – $43/hour

Daily Responsibilities for RN Field Case Manager:

• Conduct home visits to assess client needs, develop care plans

• Serve as a resource for clients, families, and staff on care coordination, program use, and community services to promote client health and safety

• Schedule: Monday to Friday, 8:00am – 4:00pm 
• No On Call, No weekends!

Required Qualifications for RN Field Case Manager:

• RN License in MN 
• Minimum 1 year of experience as an RN
• Home Health experience
