HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with Managed Care partnered SNF in New York, NY.
Daily Responsibilities for Nurse Practitioner:
- Conduct patient facing comprehensive assessments between 3 SNF facilities to prevent unnecessary hospital readmissions for acute and chronic conditions
- Conduct advanced illness and advanced care planning conversations for 4-5 patients/day
- Mon – Fri 8am-5pm
- 7-8 Month contract for understaffed region. FTE Conversions to be reviewed after 6 months.
Required Qualifications for Nurse Practitioner:
- Graduate of an accredited Master of Science Nursing program
- NY Board Certified Family, Adult, Gerontology or Adult-gerontology Acute Care NP
- DEA Prescriptive Authority
- Covid Vaccination
- 1+ years of clinical experience in NP Roles
Preferred Qualifications for Nurse Practitioner:
- Doctor of Nursing Practice
- Meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Advanced Illness and end of life discussions

Pay Details: $85 – $95 an hour
