RN Case Manager

HealthCare Support is actively seeking a RN Case Manager to fill an opening with one of the largest healthcare organ-izations in Pittsburgh, Pennsylvania.

Daily Responsibilities for RN Case Manager:
•    Ensure accurate and complete documentation.
•    Communicate and collaborate with the interdisciplinary care team
•    Assist the provider/team with various care coordination activities
•    Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D bene-fits
•    Responsible for collaborating with the primary advanced practice clinician, (APC) and facility to identify and re-spond to patient change in condition
•    Assist the provider / team with therapy coordination
•    Work hours: Monday through Friday, 8:00 am – 5:00 pm

Required Qualifications for RN Case Manager:
•    1+ year of clinical experience in practice (long-term care setting preferred)
•    Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
•    Understanding of Geriatrics and Chronic Illness
•    Proficient computer skills, including the ability to document medical information with written and electronic medical records
•    Effective Communication skills and time management
•    Ability to work with diverse care teams in a variety of settings
•    Access to reliable transportation
•    Ability to travel in between facilities

Pay Details: $52.00 – $57.00 an hour
