HealthCare Support is actively seeking a Field Patient Care Advocate to fill an opening in Atlanta/Augusta, GA with a managed care organization.
Daily Responsibilities for Field Patient Care Advocate:
- Position is to outreach to members moving to different PCP (primary care provider). May also assist with community events assisting members. Scheduling members for appointments and events. Additional need due to member moves. May have more positions/Senior PCA’s in the future.
- Will be handling member outreach to close HEDIS-related care gaps – scheduling appointments. May have some field work to provider’s offices to conduct face-to-face member education, as well as some field work for community events
- Working with Medicare members to ensure care gaps are closed, barriers to care are removed, and improve member and provider experiences.
- Location: field in Atlanta/Augusta, GA
- Shift: Monday-Thursday, 8-5 EST, Fridays 8-12 EST (36 hours a week)
Required Qualifications for Field Patient Care Advocate:
- Bachelors (or will take equivalent experience)
- Strongly prefer LPN, Certified Dietitian, or Certified Health Coach
- 2+ years of experience in patient outreach/health education experience from another MCO
- 1 year of experience in HEDIS experience
- Good tenure in past positions, 1-2+ years in prior roles
- WFH capability – smart phone, high speed internet, private work space
- Own personal transportation, active DL and insurance, ability to pass DMV report

Pay Details: $28 – 32.50/hr, depending on licensure and experience
