HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with a Fortune 500 Healthcare Organization in Portland, OR – Washington, Multnomah counties.
Daily Responsibilities for Nurse Practitioner:
Principal Duties and Responsibilities:
- Conduct comprehensive assessments, patient facing
- Effectively manage acute and chronic medical and behavioral conditions
- Ensure accurate and complete documentation, including ICD 10 conditions.
- Communicate and collaborate with the interdisciplinary care team
- Conduct advanced illness and advanced care planning conversations
- Provide patients and caregivers with counseling and education
- M-F: 8a-5p.
Required Qualifications for Nurse Practitioner:
- Clear and active NP license
- Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:
- Family Nurse Practitioner
Adult Nurse Practitioner
Gerontology Nurse Practitioner
Adult-Gerontology Acute Care Nurse Practitioner - 1 year of experience
- COVID Vaccination
Preferred Qualifications for Nurse Practitioner:
- At least 1-2+ years of experience

Pay Details: $75.00 – $90.00 an hour
