Position Summary:
A large healthcare organization is looking for a Patient Care Coordinator. This candidate will be assessing and creating health based plans for patients. Acute care experience is need for this position.
Hours for this Position:
Monday – Friday 8:00-4:30pm

Required Skills
What We Look For:
- Must have clear and active RN license in CA
- BSN or equivalent education/experience in nursing or health related field
- Acute care case management required (minimum 1 year within the last 3 years)
- Previous case management experience and demonstrated experience in utilization management, discharge planning, or transfer coordination
- Knowledge of Nurse Practice Act, The Joint Commission and other federal/state/local regulations
- Current BLS (Basic Life Support) required
Responsibilities
MAJOR JOB DUTIES AND RESPONSIBLIITIES:
Outreach to patients to establish case management, determine medical necessity, utilization review, and coordinate discharge planning and transfers. Manage and maintain relationships with patients to ensure proper care, and health is being improved.
