Position Summary:

Conduct Field Case Management in any of the following locals; Chino, Riverside, Walnut and Corona California areas of elderly, Medicare / Medicaid Health Plan Members

Home Based- and Field involved with face to face assessments and care management of health plan members

Hours for this Position:

M-F 8-5

Required Skills

What We Look For a RN Field Case Manager:

• Current Active California RN License
• Valid California Driver’s License
• Strong Computer skills
• Home visit or field work experience

Preferred but not a MUST:

• Managed Care experience, Home Health, or Field Case management experience

• Responsible for health care management and coordination of Healthcare members in order to achieve optimal clinical, financial and quality of life outcomes
• Works with members to create and implement an integrated collaborative plan of care
• Coordinates and monitors member’s progress and services to ensure consistent cost effective care that complies with Health plan policy and all state and federal regulations and guidelines
