Nurse Case Manager

Position Summary:
Our managed care company is seeking a RN or LPN for a contract case management position. This involves working onsite at a Gainesville hospital and speaking with patients (members) over the phone and providing case management services (telephonic assessments; referring patients to community resources; facilitating transfers; etc.). This is a temporary position for a few months.
Hours for this Position: The shift is three 12’s- Saturday-Sunday-Monday 6am-6pm.
Required Skills
What We Look For:
*Must live in the Gainesville area
*Must be a Florida-licensed Licensed Practical Nurse or Registered Nurse
*Must have a minimum of 4 years’ experience as a LPN or RN
*Must have experience in case management / care management
*Ability to commit to 3 12’s/week for a temporary position
