Intro:
Are you an experienced LVN or LPN with at least one year experience in clinical or case management looking for a new opportunity with a prestigious healthcare company as a LVN Service Coordinator? Position Summary:
Identify special needs members through the completion of health screens and other resources. Work with community outreach/member advocates to coordinate member care
Hours for this Position:
Monday – Friday 8AM-5PM

Required Skills
What We Look For:
- Current and active LVN or LPN in Texas
- Experience conducting assessments on patients either telephonically or face to face
- Solid Computer skills
- Ability to multi task in a fast paced environment
