Field Case Manager- RN

Position Summary:

The Field Based Case Manager position will be traveling to member’s homes and facilities, conducting health assessments, planning and coordinating continuum of care for select members with high risk, complex medical or behavioral health conditions to promote quality and cost effective care.

 

Hours for this Position:

Full time, Monday- Friday 8am-5pm

Required Skills

What We Look For:

  • Must have clear/ active Registered Nurse, RN license
  • Knowledge/Experience: Must have at least two years clinical nursing experience.
  • Experience with Aged, Blind, Disabled, Geriatric population preferred.
  • Must have reliable transportation
  • Previous case management experience preferred

Responsibilities

More Insight of Case Manager, Registered Nurse Daily Responsibilities: 

Position Responsibilities:

  • Field Based role within the San Antonio, TX area, will be traveling 60%-70% of time the remaining time will be spent working remotely from home
  • Conducting health assessments
  • Develop, assess and adjust as necessary the care plan and promote desired outcome.
  • Coordinate services between Primary Care Physician (PCP) specialist, and other medical and non-medical providers as necessary to meet the complete medical socio economic needs of clients.
  • Provide patient and provider education.
  • Identify related risk management quality concerns and report these scenarios to the appropriate resources.
  • Data entry of assessments and authorizations into the system.
